Red Flags That Signal a Fraudulent Insurance Claim

By Larkins Investigations  ·  Kentucky · Indiana · Tennessee

Insurance fraud costs American businesses and consumers an estimated $308 billion annually. For claims adjusters, SIU teams, and corporate risk managers, spotting fraud early isn't just a cost-control measure — it's a core professional competency.

At Larkins Investigations, we work alongside insurance professionals to investigate suspicious claims across Kentucky, Indiana, and Tennessee. Over the years, we've seen the same patterns repeat. Here are the red flags that consistently signal a claim deserves a closer look.

Red Flag № 01

The Timing Is Too Convenient

One of the most reliable indicators of fraud is a claim filed suspiciously close to a major life event:

  • --A policy taken out just weeks before the incident
  • --A claim filed shortly after a missed premium payment or cancellation notice
  • --A loss that occurs right before a business closes, downsizes, or faces financial hardship

Fraudsters rarely plan far ahead. When the timing feels staged, it often is.

Red Flag № 02

The Claimant Is Unusually Familiar with the Claims Process

Most legitimate claimants are confused, emotional, and uncertain about what to do next. A claimant who immediately knows the right terminology, asks pointed questions about settlement timelines, or seems procedurally calm may have been through this before — or prepared in advance.

Run a claims history search. Multiple prior claims across multiple carriers is a pattern worth investigating.

Red Flag № 03

Inconsistencies in the Claimant's Story

Listen carefully to the initial account, then compare it to the written statement, the police report, and witness accounts. Common inconsistencies include:

  • --Injuries that don't match the described mechanism of loss
  • --Timelines that shift between interviews
  • --A scene description that conflicts with physical evidence or weather records
  • --Witnesses who corroborate the story word-for-word — scripted accounts are a red flag

Small inconsistencies often grow larger under professional scrutiny.

Red Flag № 04

Medical Treatment That Doesn't Match the Claimed Injury

Fraudulent injury claims frequently involve:

  • --Treatment from clinics known to bill aggressively or operate in high-fraud networks
  • --Injuries that are difficult to objectively verify — soft tissue, chronic pain, emotional distress
  • --A pattern of treatment that begins only after the claimant retains an attorney
  • --Bills that are disproportionately high relative to the described accident

A good investigation will examine the claimant's medical history, treatment timeline, and the reputation of treating providers.

Red Flag № 05

No Independent Witnesses — or Witnesses Who Are Too Close

In legitimate incidents, independent witnesses are common. Fraud scenarios often lack them. When witnesses do exist, pay attention to their relationship with the claimant:

  • --Family members or close friends who conveniently "happened to be there"
  • --Witnesses with their own prior claims history
  • --Multiple claimants from the same address or household

Staged accidents in particular often involve a tightly coordinated group.

Red Flag № 06

The Damage Doesn't Add Up

For property and vehicle claims, physical evidence is one of the most objective tools available. Red flags include:

  • --Damage inconsistent with the described cause — e.g., fire damage originating in multiple locations
  • --Pre-existing damage included in the claim
  • --Items reported stolen that later appear for sale online
  • --Receipts or valuations that appear altered, inflated, or fabricated

Photographs, scene reconstruction, and vendor records can quickly expose inflated or staged property losses.

Red Flag № 07

The Claimant Resists Cooperation

Legitimate claimants generally want their claim resolved quickly and cooperate fully. Be alert when a claimant:

  • --Refuses to provide a recorded statement
  • --Repeatedly delays submitting documentation
  • --Avoids authorizations for medical or employment records
  • --Becomes hostile or threatening when asked routine follow-up questions

Resistance doesn't prove fraud — but it often signals something the claimant doesn't want you to find.

Red Flag № 08

Lifestyle Inconsistencies Visible on Social Media

A claimant alleging a debilitating back injury who posts gym selfies. A "total loss" business still actively marketing on Facebook. Social media has become one of the most productive investigative tools available — and one of the most frequently overlooked by adjusters.

Public activity, check-ins, tagged photos, and status updates can directly contradict claimed injuries, losses, and limitations. A professional investigator knows how to legally document this in a way that is admissible and defensible.

Red Flag № 09

The Claim Is Filed Through an Attorney Immediately

While retaining legal counsel is every claimant's right, hiring a personal injury attorney before ever speaking to an adjuster — particularly on a minor or routine claim — can indicate premeditation. Some fraud networks operate through attorney referral pipelines that route staged accident victims directly into litigation to maximize settlement pressure.

Red Flag № 10

Prior Fraud Indicators in the Claimant's History

The best predictor of future behavior is past behavior. A professional investigation can surface:

  • --Prior fraud convictions or civil judgments
  • --A history of claims across multiple carriers
  • --Prior involvement with known fraud networks or staged accident participants
  • --Address or identity inconsistencies suggesting misrepresentation on the application

When to Call in a Professional Investigator

Adjusters and SIU teams do excellent work, but there are moments when an outside investigative firm adds critical value:

◆  Surveillance — physical capability vs. claimed disability ◆  Witness interviews — recorded third-party statements
◆  Background & asset searches — undisclosed financial pressures ◆  Social media & OSINT — legally gathered open-source intelligence
◆  Scene reconstruction — revisiting incidents with trained eyes ◆  Claims history research — cross-carrier fraud pattern identification

At Larkins Investigations, we provide insurance fraud investigation services throughout Kentucky, Indiana, and Tennessee. Our licensed investigators work directly with adjusters, SIU departments, and legal teams to build documented, defensible cases — and help you make faster, more confident claims decisions.
